REED Healthcare are working exclusively with a major Public sector organisation and recruiting for a Qualified Social Worker to work within the Aged and Disabilities division.

The purpose of the Aged & Disability team is to deliver a comprehensive and accurate home-based assessment and referral service to frail aged residents and those living with a disability and their carers. Provide care co-ordination for eligible residents when appropriate. Assist with the monitoring of the Aged and Disability Services contract for provision of Home and Community Care (HACC) Services. Contribute to service planning and continuous improvement in Aged and disability Services. Undertake a community development role through participation on local management committees, and projects.

The role will entail the following responsibilitiles:

• Information & referral
• Community Involvement
• Planning
• Multiskilling

Applicants MUST have a degree in Social Work or Psychology.

2 years minimum Aged Care and / or Disability experience

Police & Working with Children Checks

 Skills in functional/Social and emotional home based assessment of frail aged people and those living with a disability and their carers.
